Experienced Support Coordinator – Work From Home | Gold Coast

We are a fast-growing and leading NDIS Support Coordination and Specialist Support Coordination provider based on the Gold Coast. Due to continued growth, we are seeking experienced Support Coordinators to join our dedicated and innovative team.

This is a full-time, flexible work-from-home position, with the requirement that the successful applicant resides on the Gold Coast or South East Queensland to attend face-to-face meetings with participants and stakeholders as required.


Key Responsibilities

As a Support Coordinator, you will coordinate supports in line with participants’ NDIS Plans and facilitate access to services that help participants achieve their goals.

You will be responsible for:

  • Managing an assigned caseload of participants
  • Supporting participants to understand their NDIS plans and goals
  • Coordinating supports by connecting participants with suitable service providers
  • Building participants’ capacity to maintain service relationships
  • Delivering services in alignment with NDIS plans and service agreements
  • Establishing, monitoring, and reviewing NDIS budget forecasts
  • Monitoring and assessing participant progress toward goals
  • Writing and maintaining accurate reports in line with NDIS compliance obligations
  • Building strong rapport with participants and key stakeholders (families, Allied Health professionals, and service providers)
  • Maintaining regular communication with participants and stakeholders
  • Using a participant-led, strengths-based approach to support independence and confidence

About You

To be successful in this role, you will have:

  • Demonstrated knowledge and experience of the NDIS, including legislation, regulations, policies, and practices
  • Previous experience working as an NDIS Support Coordinator (essential)
  • Strong interpersonal and stakeholder engagement skills
  • Proven ability to achieve KPIs
  • Proven ability to meet administrative, reporting, and compliance requirements
  • Excellent time management and prioritisation skills
  • Strong written and verbal communication skills
  • A commitment to continuous learning and professional development
  • Proficiency in Microsoft Outlook and Teams
  • Intermediate to advanced computer literacy

Requirements

  • Minimum 2 years’ experience as a Support Coordinator
  • NDIS Worker Screening Check
  • Working with Children Check (Blue Card)
  • Current driver’s licence
  • Reliable vehicle with comprehensive car insurance
  • Current Police Check (no older than 6 months)
